Yeah my pack got lost in the post and my DS2019 form. At one stage Alexis told me i probably wouldn't be able to go to Disney coz u can only get the form once and i was devastated for two weeks, even my mum was crying. But in the end they got me another one so not to panic just make sure they know you haven't receieved it!

it has 2 luggage tags, 2 mickey stickers, a disney look book, 2 letters jus like a welcome and congratulations, insurance form thing, a list of rules and rehs and i little book on life at disney and where u will live and work etc - very exciting ;D
