US visa cheat sheets for UK participants

    well i heard that a few of you are having some problems with doing your visa forms so i've copied the stuff CCUSA gave me for when i had to fill mine in back in may, so i thought i'd give everyone here the same help as i had. so hopefully the following does its job

    Form 156 - Make sure this is filled out online!!!!

    1. Passport number or document number for the passport you are using for the visa
    2. City passport was issued - if UK participant and you don't no your issueing city London will be accepted
    3. Country of the passport you are using
    4. Date passport was issued (DD-MMM-YYYY i.e 26 MAR 2003)
    5. Date your passport expires (MUST NO EXPIRE NO LESS THEN 6MONTHS AFTER YOUR RETURN)
    6. Surname as in Passport
    7. First and middle names excatly as they are in your passport
    8. other surnames used. leave blank if you have never used any other surname
    9. Other first and middle names used. leave blank if not applicable DO NOT USE NICK NAMES
    10. Date of Birth (DD-MMM-YYYY i.e 01 feb 1986)
    11. city, country and state where you were born
    12. Nationality - country of the passport you are using for your visa
    13. Hopefully you no the answer to this and tick the appropriate box
    14. This is not your National insurance number - most people in the UK will leave it blank
    15. Full home address UK participants must have a UK address
    16. Home telephone number (do not write your mobile number here) for business number give your university number or work number, if you have a mobile write this in the space provided
    17. Marital status - tick the appropriate box
    18. if you are married write spouses name here if not leave blank
    19. DOB as above
    20. Name of your University if your a student if not your works address
    21. write student or occupation here if you are not a student
    22. Date you intend to enter the USA
    23. Email address - if you have multiple one use your most common one
    24. Disneys address here
    25. again address and phone number here
    26. the period you intend to stay in the USA don't include any grace/travel periods here
    27. Name od programme here i.e camp counselling/support staff ect
    28. Myself
    29. if you have been to the USA before (even of holiday) tick yes
    30. if you have been issued a us visa before tick yes (this doesn't include UK participants who have only been on holiday there and filled out a "visa waiver" on the plane as this is not a visa)
    31. hopefully most people will answer NO here. if you have previously been refused a visa you MUST give details here
    32. Tick YES as you do intend to work in the USA and underneath write the disney address - this must be a physical address PO BOX addresses will not be accepted
    33. Tick NO
    34. Only include close family reltions (parents,siblings and spouses) if you are going to be on the same flight and going to the same location Dont include BF/GF friends or any other person not related to you Write NONE if no ne
    35. Hopefully you will answer NO to this one
    36. most will answer No but if anyone has filled for a green card ect tick yes
    37. Tick no unless you have a brother/sister/parent or spouse legally living in the usa
    38. Read each statement hopefully you will tick NO for each question - ring yummy immediatly for advice if you tick yes to any statement
    39. you are the person completing this form tco tick NO
    40. Leave blank unless someone else completed this form on your behalf
    41. Click on the Continue button - failure to do this will result in an unacceptable form
    42. This will bring up the form you have just completed with a BARCODE on the front page above your photo area and you should have a 3rd page now.
    43. Print out all 3 pages on WHITE PAPER and with black ink - Make sure the barcodes print clearly with no lines running through them
    44. sign application and date with todays date in DD-MMM-YYYY formate.
    45. DO NOT FOLD THE PAPER - KEEP IT FLAT

    Form 157

    Print the blank form out and complete it neatkly in black ink

    1. Last name
    2. First name only
    3. Full name - yes write it again
    4. most applicants will wrie NONE
    5. if unmarried write NONE
    6 & 7 this should be you fathers and mothers sull name according to there passport
    8.Your conact in the USA including name, address and phone number
    9. Yep you have to list every single country you have visited in the last 10 years all you need to put is the country and year i.e france 1999, germany 2003 etc. Please continue on the back of the sheet
    10. Hopefully self explanatory for most people it will be the UK
    11. Agaian hopefully most will answer no to this one
    12. you sholud only include your last 2 employees this can include part time work such as babysitting and even paperrounds. if you have never work put NONE
    13. Most people will tick NO however if you have done on going charity work or make regular charity donations give details (buying the big issue doesn't count)
    14. Most people will answer NO
    15. Same as above
    16. Same as above
    17. only include schools from year 7 onwards if there is not enough room write on the back or use a seperate A4 plain white sheet of paper
    18. self explanity

    If usure on any Q's phone yummy

    Form 158

    print this on 1 sheet of A4 with page 2 on the reverse and use black ink

    1. Last name, frist name , middle name
    2. Date of birth (DD-MMM-YYYY)
    3. Country of Birth, city/town of brith, state/provice/ county of birth
    4. permanent homa address - use parents rather than your universuty
    5. If not married write NONE
    6. Here you must give the names, addresses annd telephone numbers of your parents, siblings and any children. Do not use PO box addresses is unknown write Address unknown, Phone number unknown
    7. Your should give the contact details of two people in the UK who can verify your information - they must not be related to you

    Page Two

    Here you should give the details of you pats and present jobs. For those in fiull timme educations and have not held a fulltime job it is fine to list any part time jobs you have held. if you have never hada job including casual work please write NONE. PLease complete as many work experience boxes as you possibly can!

    On the second page for the ds158... it says present job in the first box... do u just leave that blank if you dont have a job and are just at uni?

    i think so i dont have a job at the moment so i just left it blank..

    ive made some mistakes on my form oops!!! i put my national insurance number in or do u think it will be ok if i just leave it as it is?

    i shouldn't thisnk it will be to big a problem if you did put ur national insuranse number in if your that unsure you can always redo that form.

    also for the DS158 if you don't have a present job just put NONE in the box

    Ello Ello,

    On the 156 form i have put my issuing city as portsmouth now this is where i live and am not sure if this is correct. i dont know my issuing city so do i need to change it to London?

    Secondly, i have put my NI number in question 14. is this ok or do i need to change this to?
