It's probably the one you can get off the official website....I think you could send that one or just your cv.....but it doesn't matter....if anything emma, you'll have no problem filling it in cos you've done it already! Ellen xxx

I got mine yesterday. Hope my birth certificate arrives in time- having to get my mumto send it down. Oh and looks like I'll have to find a lot of photocopying to do at work all of a sudden
yeah, I know what you mean! mom's coming to my uni place to give me the baby for a couple of days and she has to bring up my passport and birth certificate too.

I reckon, bearing in mind how many of us seem to want to go out for a meal, we shouldn't go too late, or they might not be able to seat us for ages.... I reckon I'll be getting there about 4/5-ish....that's not definite......anyone else got an ETA? Ellen xxx
Depending how many of us, coz there seems to be a lot, should we try to get def amount of people who want to go then book a table? Then a least we know what time we have to be there for, so could meet up with plenty of time to get there. Helen x
